Bacteriostatic water is water with 0.9% benzyl alcohol, and the benzyl alcohol is the reason it exists and the reason it carries a warning. The preservative lets a multi-dose vial be punctured repeatedly without growing bacteria; it is also the agent that caused the “gasping syndrome” — metabolic acidosis, respiratory depression and death — in premature neonates given benzyl-alcohol-preserved flush solutions in the early 1980s (Gershanik et al., 1982, NEJM). In mice the acute toxicity is due to the alcohol itself, not its metabolite benzaldehyde, with an LD₅₀ near 1 g/kg (McCloskey et al., 1986, Journal of Pharmaceutical Sciences).
A diluent is only useful if it is sterile and its preservative is at the stated concentration. Every batch is sterility- and endotoxin-tested and assayed for benzyl alcohol content.

About the compound
The product is purified water containing 0.9% w/v benzyl alcohol as a bacteriostatic preservative, sterile-filtered, filled into 10 ml multi-dose vials and terminally sterilised. Benzyl alcohol at this concentration inhibits bacterial growth in a vial that will be entered more than once; sterile water for injection without preservative is single-use.
It is supplied for reconstituting lyophilised peptides for laboratory use: solubility testing, analytical sample preparation, in-vitro assays. Benzyl alcohol can affect the stability of some peptides and interfere with some assays, and for those cases a preservative-free diluent is the correct choice.
The neonatal hazard is the reason the preservative is never used in products for premature infants: the 1982 reports linked benzyl-alcohol-preserved solutions to a syndrome of gasping respirations, metabolic acidosis and death in low-birth-weight infants (Gershanik et al., 1982). This product is not a medicine and is not for administration to humans or animals.

Toxicity in adult and neonatal mice. Intraperitoneal benzyl alcohol: minimal effects below 800 mg/kg over 4 hours; LD₅₀ 1,000 mg/kg in both age groups at 4 hours and 650 mg/kg at 7 days in adults; toxicity attributable to benzyl alcohol rather than benzaldehyde (McCloskey et al., 1986).
