RAD140 is the only compound in this class to have been given a proper drug name. It is now vosilasarm, and it has been through a first-in-human oncology trial — which makes it the most clinically advanced SARM after ostarine, and the one with the clearest liver signal in the published record. In that Phase 1 study, 59.1% of participants showed elevated AST and 45.5% elevated ALT. Both numbers belong on the same page as the efficacy data.

About the compound
RAD140 is a non-steroidal, orally active selective androgen receptor modulator, first described by Radius Health and later licensed to Ellipses Pharma as EP0062. It binds the androgen receptor with high affinity and acts as an agonist in muscle and bone while showing markedly weaker activity in prostate tissue — the tissue selectivity the class is named for.
Its research history has run along two separate tracks. The oncology track pursued RAD140 as an androgen-receptor-targeted therapy for AR-positive, ER-positive breast cancer, on the logic that AR agonism can suppress ER-driven tumour growth. The musculoskeletal track looked at anabolic and neuroprotective effects: RAD140 was reported to be neuroprotective in cultured neurons and in kainate-lesioned male rats (Jayaraman et al., 2014, Endocrinology).

What the published research shows
First-in-human Phase 1, metastatic breast cancer. Twenty-two heavily pretreated postmenopausal women with ER+/HER2− metastatic breast cancer, 21 of them AR-positive, received RAD140 at 50 mg (n=6), 100 mg (n=13) or 150 mg (n=3) once daily. The maximum tolerated dose was 100 mg/day. One patient with a baseline ESR1 mutation had a partial response; the clinical benefit rate at 24 weeks was 18.2% and median progression-free survival 2.3 months (NCT03088527).
